- We’ve provided copies of our new patient information forms for you to download, print, and complete at home to bring to your next scheduled appointment. If you have not completed the form ahead of time, kindly arrive 15 minutes before your scheduled appointment.

What To Expect
- If you have previous x-rays or other diagnostic studies that would aid the doctor in your treatment please bring them with you.
- Please also bring the shoes that you most commonly wear as well as any orthotic devices you may have used.
